Maui Training Center Liability Waiver Form

Now for some legal stuff.

1. Assumption of Risk: I understand and acknowledge that the use of the facilities and participation in activities at Maui Training Center involves inherent risks, including but not limited to the risk of injury, illness, or death. I voluntarily assume all such risks.

2. Release of Liability: In consideration for being permitted to use the facilities and participate in activities at Maui Training Center, I hereby release, waive, discharge, and covenant not to sue Maui Training Center, its owners, employees, agents, and affiliates from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my use of the facilities or participation in activities, including any negligence on the part of Maui Training Center.

6. Indemnification: I agree to indemnify and hold harmless Maui Training Center, its owners, employees, agents, and affiliates from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees, arising out of or in any way related to my use of the facilities or participation in activities.

I have carefully read this waiver and fully understand its contents. I am aware that by signing this waiver, I am waiving certain legal rights, including the right to sue. I voluntarily sign this waiver of my own free will.
